Health Form Disclosure

Proper Title is also requiring all parties coming in any manner of physical proximity to our facilities and closers to complete a disclosure form that asks questions about whether the attendee has been exposed to COVID-19 or whether they, or someone close to them has experienced symptoms. 

We are asking for the form on curbside closings and document drop-off as well.  We maintain the privacy of  this form, but also take it very seriously. 

We feel the disclosure is important for the comfort and safety of our closers and thus far, our customers have been very cooperative and supportive of this requirement.
